A BBL (Brazilian Butt Lift) transfers your own fat to reshape and augment the buttocks in a single surgery. Buttock fillers add volume with injectable gels or collagen stimulators, no surgery required. Neither is “better”: they solve different problems. What Each Option Actually Is

The BBL: your own fat, placed where it belongs. A Brazilian Butt Lift is a two-part surgery. First, liposuction harvests fat from areas where you have excess, typically the waist, flanks, or thighs. That fat is then purified and injected into the buttocks to add projection and reshape the whole region. Because the contouring and the augmentation happen together, the result is more than added volume: the waist-to-hip transition changes, which is what creates the shape patients actually want.

Worth knowing: PLLA is FDA-approved for facial use, and its application in the buttocks is a widely practised off-label use, one more reason product choice and injector training matter.

Advantages and Disadvantages, Honestly

BBL advantages: the largest possible change, with real projection and a reshaped silhouette. The liposuction is half the result: a defined waist makes the proportions work. It uses your own living tissue, nothing synthetic remains, and the surviving fat is permanent, making it a one-time investment.

BBL disadvantages: it is surgery, with general anesthesia and a surgical recovery. Expect 10 to 14 days of social downtime and a modified sitting protocol for about two weeks. You need enough donor fat, so very slim patients may not be candidates. Some transferred fat is reabsorbed, and final results take about three months to settle.

Filler advantages: no surgery, no general anesthesia, no meaningful downtime. Precision makes fillers ideal for hip dips, mild asymmetry, and subtle projection. They work for slim patients who lack donor fat, the result builds gradually and is adjustable, and HA is reversible.

Filler disadvantages: a volume ceiling. Fillers refine, they do not transform, and a dramatic change is not realistic. Results are temporary, so maintenance sessions are part of the plan, and the per-millilitre economics become poor if you chase large volumes. There are small risks of nodules or delayed reactions, which is why product quality and sterile technique are non-negotiable. Fillers are usually the right answer when: you are very slim with genuinely insufficient donor fat; your goal is correcting hip dips, mild asymmetry, or adding a discreet amount of projection; downtime is impossible for you right now; you want to preview a subtle change before considering anything definitive; or a medical reason makes surgery inadvisable.

The two also work together. Some patients have a BBL for the structural change, then use small amounts of filler a year later to fine-tune a hip dip or minor asymmetry. Done in the right order and with proper planning, the combination is safe and effective.

Safety: Where the Hard Lines Are

For BBL surgery: ultrasound guidance, always. Visualising the cannula in real time and keeping fat above the muscle is what made the modern BBL a responsible operation. Dr. Bravo does not perform this surgery without it.

For fillers: only regulated, CE-marked products, injected by qualified medical professionals. The injectable market attracts dangerous shortcuts: liquid silicone, so-called biopolymers, and unregulated substances cause permanent disfigurement and are sadly common in injection tourism.
